Consent To Participate in MUMA Services:

I hereby consent to actively participate in Manukau Urban Māori Authority (MUMA) Whānau Services with a dedicated Kaiārahi assisting my whānau and I to address our concerns and social issues collaboratively;

I have been made aware of my rights and the complaints procedures whilst receiving Whānau Services;

I authorise my Kaiārahi to enter my property for the purpose of providing my whānau and I with social services at times agreed to between myself and my Kaiārahi. I also commit wholeheartedly to fulfil all requirements expected of my whānau and I in order to achieve our agreed outcomes and goals once these have been determined in our Whānau Plan;

I authorise MUMA to disclose relevant and necessary information to any other party concerned and involved in the best interests of my Whānau well-being and achieving our outcomes and goals according to our Whānau Plan;

I acknowledge that MUMA may use information about my whānau and I for general statistical or reporting purposes, as long as it is not published in a form that could identify my whānau or I;

I understand that I can withdraw my consent to MUMA Services specific at any time and that this may affect the services provided and this will be discussed directly with my Kaiārahi first.

I also understand that I can request access to the information held about my whānau and I by requesting this from my Kaiārahi.
